Community Health & Wellness Partners is a non-profit health center that is changing lives in our community by eliminating barriers to care. We provide patient-centered care to anyone in Logan or the surrounding counties. Our primary care providers and other health care professionals are passionate about our mission and the community and families we serve at one of three locations in Logan County — in Bellefontaine, Indian Lake and West Liberty.
