Community Health & Wellness Partners has an immediate need for a full-time Patient Care Assist. All providers at Community Health& Wellness Partners serve the mission of providing high-quality, whole-person, patient-centered medical care to anyone and everyone in our community.

As a Federally-Qualified Health Center, with integrated ancillary services, we are looking for a Patient Care Assist to change lives by eliminating barriers and providing a standard of health care that improves the well-being of the whole person. Our organization believes that we are about our patients, that we are changing lives in our community, that passion is a part of our core and that we have a strong commitment to outstanding patient care. We believe that our approach to care is raising the bar for providing high-quality patient care throughout the region. Organizational certifications include NCQA Patient -Centered Medical Home and Diabetes Education Accreditation Program.

Community Health and Wellness Partners (CHWP) is an integrated primary-care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. The Patient Care Assistant is responsible for maintaining an excellent attitude in participating in the day-to-day clinical operations of the health center under the guidance and support of the Clinical Coordinator. Responsible for the collection of data, administration of medications and treatments, and documentation of care provided as qualified. Support the coordination of all activities of team members engaged in clinical care for patients under the directions of the Clinical Team Lead, Clinical Coordinator and/or Providers. Responsible for training like staff in team-based care and supporting team members in staying focused on the organization’s Mission, Vision and Values.
