Community Health & Wellness Partners has an immediate need for a full-time Outreach Associate. All employees at Community Health & Wellness Partners serve the mission of providing high-quality, whole-person, patient-centered medical care to anyone and everyone in our community.

As a Federally-Qualified Health Center, with integrated ancillary services, we are looking for an Outreach Associate to change lives by eliminating barriers and providing a standard of health care that improves the well- being of the whole person. Our organization believes that we are about our patients, that we are changing lives in our community, that passion is a part of our core and that we have a strong commitment to outstanding patient care. We believe our approach to care is raising the bar for providing high-quality patient care throughout the region. Organizational certifications include NCQA Patient -Centered Medical Home and Diabetes Education Accreditation Program.

Candidate must function as often the first point of contact for all patients using our facilities for health care. Manage the timely and accurate collection of important patient information and function as a resource to patients entering the facility. Support and act as an advocate for patients by assisting them with the registration process. Conduct outreach efforts and implement financial assistance programs of CHWP.

Bilingual in Spanish is helpful, but not required. Associate degree, Medial Assistant certification and/or two-years-related experience helpful, but not required. Please send cover letter and resume to humanresources@chwplc.org
