Clinical Team Lead
Community Health & Wellness Partners has an immediate need for a full-time Clinical Team Lead. All providers at Community Health& Wellness Partners serve the mission of providing high-quality, whole-person, patient-centered medical care to anyone and everyone in our community.
As a Federally-Qualified Health Center, with integrated ancillary services, we are looking for a Clinical Team Lead to change lives by eliminating barriers and providing a standard of health care that improves the well-being of the whole person. Candidate must be a Licensed Registered or Practical Nurse with a high level of skill at building relationships and customer service. Demonstrate interpersonal savvy and influence skills in managing difficult clients and patients. CHWP is an integrated, primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. In addition to providing relevant nursing and support services to patients, the Clinical Team Lead provides leadership by maintaining an excellent attitude using leadership by example, the Clinical Team Lead provides relevant nursing care under the guidance and supervision of a Clinical Coordinator (RN). Responsible for the collection of data, administration of medications and treatments, and documentation of care provided. Supports the coordination of the activities of all team members engaged in clinical care for patients under the direction of the Clinical Coordinator and Providers. Manages all emergency patients, responsible for training staff in team-based care and supports the management of team members to stay focused on the organization’s Mission, Vision and Values.
