Community Health and Wellness Partners, a Federally Qualified Health Center, is looking for its newest team member! The Behavioral Health Case Manager serves as a liaison within the center by managing primary care services such as: Assisting with social service needs, providing patients with brief interventions for various health concerns, developing integrated care plans, connecting patients to resources and coordinating referrals to ensure quality care is achieved for the whole person, whole community. Bilingual in Spanish is helpful, but not required. Applicants should hold a bachelor’s degree in social work, an LSW is preferred. One to three years clinical case management experience or experience in community‐based behavioral health care is highly desired.
