Candidate must be a Licensed Registered Nurse with a high level of skill at building relationships and customer service. Demonstrate interpersonal savvy and influence skills in managing difficult clients and patients. CHWP is an integrated primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. In addition to providing relevant nursing and support services to patients, the Clinical Coordinator is a team leader responsible for all clinical operations within the team, including but not limited to: serving as an extension of the provider/s in the coordinating and managing patient flow and quality improvement; supporting human resources with the process of personnel management; managing conflict within the team and/or patients; developing training/cross-training systems; and assisting and supporting the management of team members to stay focused on the organization’s Mission, Vision and Values.
