Are you interested in integrated behavioral health in a primary care setting? Community Health & Wellness Partners of Logan County has an exciting opportunity for a driven, organized individual to collaborate with our primary care and psychiatric providers. CHWPLC provides economic‐based whole person, whole community care to patents for long‐term health and wellness. Candidates must be a LPCC, LISW‐S or LISW, or MSW with an LSW working toward LISW preferably with two‐ to five‐year’s experience working with substance use and mental health disorders in outpatient therapy/counseling; hold an active Ohio license; and can work collaboratively and independently with primary care providers.
