Triage Nurse
Community Health & Wellness Partners has an immediate need for a full-time Triage Nurse. All providers at Community Health& Wellness Partners serve the mission of providing high-quality, whole-person, patient centered medical care to anyone and everyone in our community.
As a Federally-Qualified Health Center, with integrated ancillary services, we are looking for a Triage Nurse to change lives by eliminating barriers and providing a standard of health care that improves the well-being of the whole person. Organizational certifications include NCQA Patient-Centered Medical Home and Diabetes Education Accreditation Program.
Candidate must be a Licensed Registered Nurse or a Licensed Practical Nurse with a high level of skill at building relationships and customer service. Demonstrate interpersonal savvy and influence skills in managing difficult clients and patients. Community Health and Wellness Partners (CHWP) is an integrated primary care community health center that utilizes a team-based care model, sharing patient-care responsibilities among members of a team. The Team Triage Nurse performs telephone triage and addresses urgent and non-urgent walk-in triage for an assigned Team. Provides advice/guidance to patients/families of all ages by using algorithms/protocols and independent judgement as directed by Team Providers. Ranks patients according to urgency of need, directing them to further care, as well as directing the patient to the appropriate emergency services, when necessary. Recommends the suggested medical follow-up based on their assessments and established triage protocols/algorithms, and provides health care information/education, as necessary. Assists and supports the team members to stay focused on the organization’s Mission, Vision and Values.
